Occupational Therapist – Discharge to Assess

Liquid Personnel is currently recruiting for an experienced Occupational Therapist to join the Discharge to Assess Team at the Redbridge.

This is an excellent opportunity to join a supportive, locality-based multidisciplinary team delivering integrated community health and Adult Social Care services. You'll play a key role in supporting safe hospital discharges while promoting independence through high-quality, person-centred Occupational Therapy interventions.

The Role

As an Occupational Therapist within the Discharge to Assess Team, you'll manage a caseload of approximately four cases per week, undertaking both standard and complex assessments for adults with a wide range of health needs.

Working autonomously and alongside multidisciplinary colleagues, you'll assess for specialist equipment, manual handling solutions, and major adaptations to ensure people can return home safely and maintain their independence.

Key Responsibilities

  • Undertake holistic, person-centred Occupational Therapy assessments.
  • Assess for and recommend simple and complex equipment, manual handling solutions, and major adaptations.
  • Complete support planning and reviews for adults and their carers.
  • Manage and prioritise a varied caseload of complex and standard cases.
  • Work collaboratively with health, housing, and social care professionals.
  • Provide advice and training on the safe use of equipment in line with legislation and best practice.
  • Maintain accurate case records and contribute to safe discharge planning.

Essential Requirements

  • HCPC-registered Occupational Therapist.
  • Extensive experience assessing for complex equipment, specialist equipment, manual handling, and major adaptations.
  • Experience working within a multidisciplinary team undertaking complex Occupational Therapy assessments.
  • Strong knowledge of Adult Social Care legislation and housing legislation.
  • Proven ability to independently manage, prioritise, and progress complex cases.
  • Experience reviewing equipment provision and educating service users and carers on safe use.
  • Excellent communication, organisational, and clinical reasoning skills.
