Liquid Personnel is recruiting an experienced Occupational Therapist for its client’s Hospital Discharge Team based in North London.
This is a full-time agency role offering £291.70 per day, with hybrid working arrangements and flexibility between home and office-based working.
About the Role
Our client is seeking a skilled and motivated Occupational Therapist to join a busy Hospital Discharge Team, supporting adults to achieve safe, timely, and effective transitions from hospital back into the community.
This is an excellent opportunity to work within a multidisciplinary setting where you will use your clinical expertise to promote independence, reduce delayed discharges, and help individuals achieve the best possible outcomes following hospital admission.
You will play a key role across the hospital discharge pathway, completing specialist Occupational Therapy assessments, identifying appropriate interventions, and supporting people to return to the least restrictive environment possible.

What You Will Be Doing
- Completing specialist Occupational Therapy assessments in accordance with the Care Act 2014.
- Supporting safe and effective hospital discharges through a Discharge to Assess approach.
- Assessing functional abilities, risks, equipment requirements, and adaptation needs.
- Recommending appropriate equipment, assistive technology, and therapeutic interventions.
- Supporting individuals to maximise independence and wellbeing following hospital admission.
- Developing outcome-focused support plans that build on strengths and reduce dependency on long-term services.
- Providing Occupational Therapy input into multidisciplinary discharge planning meetings.
- Working collaboratively with social workers, NHS professionals, therapists, and community-based services.
- Providing hospital duty cover and responding to urgent referrals impacting discharge pathways.
- Completing timely assessments to support hospital flow and reduce discharge delays.
- Offering professional advice and guidance on Occupational Therapy practice and risk management.

What We Are Looking For
To be successful in this role, you will have:
- A recognised Occupational Therapy qualification.
- Current HCPC registration.
- Previous experience within Hospital Discharge, Adult Social Care, Community Occupational Therapy, or a similar setting.
- Strong knowledge of Care Act assessments and strengths-based practice.
- Experience working within multidisciplinary teams.
- The ability to manage urgent referrals and complex cases effectively.
- Excellent assessment, communication, and report-writing skills.
