Occupational Therapist – Integrated Cluster Network Teams (Full-Time Permanent)
Hours & Location
- 37 hours per week
- Base location: Bridgend (with county-wide coverage)
- Primary office: Glanrhyd
- Hybrid working: Complies with the Council’s Hybrid Working Policy
About the Role
This is an exciting opportunity to join our Integrated Cluster Network Teams. These teams provide holistic support to individuals—both older and younger adults—with long-term, complex challenges, including physical health, mental health, and emotional well-being. Our mission is to empower people to live autonomously, ensuring voice, choice, independence, and control in their daily lives.
Edited to contribute to a multi-disciplinary team comprising:
- Social Workers
- District Nurses
- Occupational Therapists
- Physiotherapists
- Dietitians
- Speech and Language Therapists
- Pharmacy Specialists
- Community Psychiatric Nurses
The team works closely with GP surgeries, third sector partners, and local agencies to assess and support adults with long-term, complex conditions—as well as their carers and families.
Key Responsibilities
The Occupational Therapist (OT) will:
- Apply a strength-based, outcome-focused model to support individuals and carers in achieving positive life outcomes.
- Specialise in maximising independence for those receiving managed care support, ensuring commissioned care is proportionate to needs.
- Prioritise targeted reablement goals for individuals requiring long-term recovery.
- Work under the Contingency Care Model to prevent unnecessary hospital admissions or care home placements.
- Employ specialist clinical skills, evidence-based assessments, and outcome-driven interventions to refine care plans.
- Develop and maintain strategic partnerships with statutory services, third-sector agencies, and community groups.
- Collaborate with Adult Social Care professionals to implement outcome-focused care packages.
- Foster local community networks, support groups, and services in Bridgend.

Requirements
Essential Criteria
- Qualified Occupational Therapist (minimum Health & Care Professions Council registration) with evidence of strong clinical practice and outcome management.
- Ability to conduct detailed, evidence-based assessments tailored to individual needs.
- Proficiency in working across multi-agency teams in a collaborative, partnership-driven model.
- Commitment to preventative interventions, aiming to reduce care dependency.
- Ability to champion high-quality OT practice.
- Confident ability to work autonomously within a specialist care area while also absorbing knowledge to support broader team practices.
- Valid UK driving licence for travel between home and office or to client visits.
- Enhanced Disclosure & Barring Service (DBS) check with Adults Barred List, at a cost covered by the employer.
- Mandatory safeguarding awareness—proactive responsibility towards protecting children, vulnerable adults, and at-risk individuals.
- Mandatory requirement to communicate effectively in both English and Welsh.
