Occupational Therapist – Reablement Service
Chippenham, Wiltshire | Ongoing Temporary Contract
Service Care Solutions are currently recruiting for an Occupational Therapist to join a Reablement Service within a Local Authority based in Chippenham, Wiltshire.
This is an excellent opportunity for an Occupational Therapist looking to support adults to regain independence and remain safely in their own homes following illness, injury, or hospital discharge.
Role Details
Rate of Pay:
- £36.00 per hour (Umbrella)
- £28.78 per hour (PAYE)
Contract: Temporary Ongoing Location: Chippenham, Wiltshire Hours: Full-Time

Key Responsibilities
- Undertake Occupational Therapy assessments within the community and reablement settings.
- Support adults to maximise independence and achieve personal goals.
- Recommend appropriate equation, adaptations, and moving and handling solutions.
- Develop and review reablement plans to promote safe and effective discharge from hospital and prevent admissions.
- Work closely with multidisciplinary teams, carers, and community services to deliver positive outcomes.
Requirements
- Degree or Diploma in Occupational Therapy.
- HCPC Registration.
- Previous experience within Reablement, Community OT, Adult Social Care, or Intermediate Care services.
- Strong assessment and care planning skills.
- Full UK Driving Licence and access to a vehicle preferred.
