Occupational Therapist/ Senior Physiotherapist
About the Role
We are looking for an experienced, motivated clinician to join our team. You will be a registered Occupational Therapist/ Senior Physiotherapist with demonstrable expertise in discharge planning, supporting patients to become experts in their own care and to remain in their own homes.
Key Responsibilities
- Provide rapid assessments in a fast-paced environment.
- Formulate robust discharge plans.
- Work with the wider MDT to problem solve and support patients into their preferred place of care.
Requirements
- Excellent communication skills.
- Enjoy working with a variety of service users of different age groups.
- Good self-management skills and an ability to prioritise your case load.
- Experience of working in a variety of clinical settings with sound medical, respiratory, and social knowledge.
Working Conditions
- 7 day working in a 4 week pattern, with x2 days off prior to or after the weekend.
- Periods of requiring you to work 10:00-18:00 Mon- Friday once every 8 weeks.
