Exciting Opportunity for a Band 6 Community Care Coordinator
An exciting opportunity has arisen for a Band 6 Community Care Coordinator to join our lively and friendly South and West Community Mental Health Team for Older People.
About the Role
Based in the multicultural city of Bradford, we are seeking skilled, enthusiastic, and compassionate clinicians to deliver outstanding care to our diverse community.
Covering both South and West areas, we take pride in serving one of England’s most culturally rich populations and continually explore innovative, inclusive, and person-centred ways of working that celebrate diversity and foster a welcoming team culture.

Key Responsibilities
- Support clients through challenging times, empowering them toward recovery and independence.
- Adopt a flexible, open-minded approach, ensuring all interventions align with care pathway frameworks and national standards.
- Support the ever-developing Memory Assessment Service and participate in the pre-diagnostic assessment pathway.
- Manage a caseload, coordinate holistic care, and deliver recovery-focused interventions within a supportive multidisciplinary team.

About You
We seek caring, creative professionals who thrive under pressure, connect well with others, and are passionate about helping people live independently. As a band 6 clinician, you will be expected to act as a role model and resource for our less experienced staff. You will also have the opportunity to be involved in service delivery improvements and innovations.
