HCSW – Band 3 Operational Discharge and Flow Support
Hampshire Hospitals NHS Foundation Trust is expanding its Operational Discharge and Flow Support Team, newly formed by merging discharge and transfer teams across Winchester and Basingstoke sites. Exciting Band 3 HCSW roles are available—read on to learn more.
About the Role
The Operational Discharge and Flow Support Team collaborates with inpatient teams, clinical departments, Integrated Discharge Teams, and operational centres to enhance patient care and operational flow. Key objectives include:
- Enabling earlier discharges via the Discharge Lounges.
- Reducing ED and assessment area delays to improve patient transfer speed.
- Tackling blockages across service lines through proactive support.
This dynamic role requires flexibility, adaptability, and teamwork under the guidance of senior staff (Band 7 and Site & Duty teams).

Core responsibilities:
- Coordinating discharges with the Integrated Discharge Team, Patient Flow Facilitators, wards, and the Operations Centre.
- Maintaining accurate data on Ewhiteboards.
- Facilitating transport arrangements.
- Supporting patient engagement for safe, timely transfers—both at admission and discharge.
- Quick response and flexibility across duties.
Key Requirements
- Results-driven mindset with problem-solving skills.
- Ability to work collaboratively in fast-paced environments.
- Effective communication (both verbal and digital).
- Proficiency with electronic patient systems (Ewhiteboards).
- Practical approach to resolving transit delays.
