About the Role: HALT Team (Home at Last)
Join the HALT Team – supporting adults who are concerned about rough sleeping, helping them access treatment and accommodation across Sheffield.
This role is part of a multi-agency approach, working closely with housing providers, homelessness services, and other professionals to deliver coordinated outreach support.
As a member of the team, you’ll engage with individuals in the city centre and across wider areas, offering outreach-based interventions for people experiencing multiple disadvantage:
- Complex challenges, including homelessness and substance use
- Disengagement from traditional services due to vulnerabilities
- Need for intensive support to access stability

Core Responsibilities
As directed by the Project Manager and Lead Practitioner, you will:
- Deliver outreach interventions to individuals rough sleeping or at risk of homelessness
- Support people in accessing:
- Accommodation
- Treatment services
- Wider support initiatives
- Collaborate with:
- Housing providers
- Homelessness teams
- Partner agencies
- Promote harm reduction and public health approaches, where appropriate
- Contribute to reducing multiple disadvantage and improving service user outcomes
This role includes weekend shifts and occasional evening work.
About You
To apply for this role, we are seeking candidates with the following experience and skills:
- Proven experience engaging effectively with people who have complex needs
- Good understanding of the challenges faced by individuals experiencing homelessness, substance use, and/or mental health struggles
- Experience with service user assessments, including risk assessments
- Ability to work collaboratively within a multi-agency environment
- Strong IT skills
- Must hold a full driving licence and have access to a vehicle
(Note: Due to the nature of this role, specific role experience is encouraged but not always mandatory. We are particularly open to recruitment from people with lived experience.)
