Job Description
Role Overview
To support the Paediatric Diabetes Service Nurse (PDSN) and Paediatric Diabetes service with the care, assessment, and prompt treatment of children and young people (CYP) presenting to Bedford Hospital NHS Foundation Trust with Diabetes-related conditions. To manage care provision within the hospital and community setting, and promote admission avoidance, timely discharge, and ensure that care provision is in line with evidence-based guidelines.
Key Responsibilities
- Management of Young Patients with Diabetes: Focus on initiating treatment and maintaining high standards of clinical excellence.
- Caseload Management: Active caseload management skills, self-development, and motivation.
- Guidance and Support: Guiding and supporting ward staff.
- Collaboration: Work with the Diabetes Team in collaboration with the current PDSN and Consultant(s) with an interest in Diabetes.
- Supervision: Work under the supervision and guidance of the PDSNs, the Matron for Paediatrics, and Head Nurse for Children's Services.
- Multidisciplinary Team: Act as an integral part of the multidisciplinary team within the paediatric diabetes service setting.
Clinical Responsibilities
- Outpatient Diabetes Management Clinics: Participate in outpatient diabetes management clinics under the supervision of the PDSN to assess patients and provide appropriate management plans.
- Acute Ward Support: Attend the acute ward and support the ward teams with the admission of patients with Type 1 Diabetes (T1D).
- Diabetes Technologies Service: Work within the Team to support the diabetes technologies service, keeping up with new developments and technologies through liaison with company representatives and peers.
- Hazardous Materials: Appropriately respond to daily exposure to hazardous materials such as blood and body fluids and a working environment in which the work pattern is frequently unpredictable.
- Transition to Adult Services: Engage with pathways of transition to adult services for all young people in the service.
- Individual Management Plans: Develop, implement, and regularly review individual management plans.
