An opportunity has arisen for a Patient Flow/Discharge Coordinator to join the East Mental Health & Learning Disabilities (MHLD) Team based at the Heddfan Unit in Wrexham. The Patient Flow role within the East is unique to MHLD but a well-established function across the Health Board, designed to support safe and effective operational management of inpatient capacity.
Main duties of the job
The successful candidate will lead and manage patient flow to ensure a seamless service is provided to ensure safe and effective discharge planning of mental health patients within the East locality. You will have substantial experience as a complex discharge nurse and be able to coordinate and supervise the team to ensure a patient-centred service.
This is an exciting and challenging role for an experienced individual who is dynamic, possesses strong problem-solving skills, with the ability to deal with difficult and sensitive situations within a busy and challenging clinical setting.
The post holder must have an extensive knowledge and understanding of inpatient, community services and discharge processes. They will need to be an experienced Trusted assessor with excellent communication, assessment and documentation skills and be committed to working within a multidisciplinary environment.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Qualifications & Knowledge
Essential criteria
- Registered Mental Health nurse 1st Level. RMN
- Evidence of ongoing professional development and training.
- Degree or Diploma Registered nurse 1st Level.
- NMC Registration.
- Evidence of nursing related qualification to diploma or degree level
- Post graduate diploma or equivalent level experience
Desirable criteria
- Experience within the adult /Older Person Mental Health Field.
- Teaching /assessing qualification to Postgraduate Diploma Level
Experience
Essential criteria
- Post holder would need some experience in discharge planning, continuing health care eligibility criteria, and the legal aspects of discharge pathways.
- Working knowledge of being part of multi-disciplinary team.
- Clinical credibility
- Experience of working as part of a team as well as working autonomously.
