Mental Health Support Worker – Primary Care Role within the Living Well Network Alliance
Location: Agreed GP locations across Lambeth, supported by Living Well Centres and Primary Care Networks
About the Role
The Living Well Network Alliance (LWNA) is transforming adult mental health services in Lambeth by integrating community mental health teams, social workers, and the voluntary sector into three Living Well Centres. This collaborative approach delivers holistic, personalised support to service users while fostering career growth for staff.

Our organisation supports diverse specialities, with opportunities to develop into roles such as PCN Team Leader or other vacancies across the Living Well Network and South London and Maudsley (SLaM) NHS Foundation Trust.
Responsibilities
Your role will include:
- Patient & GP Engagement:
- Collaborate with patients and General Practice teams to guide them toward appropriate mental health services, including referrals to Living Well Centres or signposting to social prescribers, voluntary sector services, and community support groups.

Early Intervention & Self-Management:
- Promote shared decision-making and self-management of symptoms to enhance treatment adherence and patient autonomy.

Improving Service Pathways:
- Contribute to streamlined communication and collaboration between primary and secondary care to refine care pathways.

Physical Health Integration:
- Support physical health improvement for individuals with mental health needs through holistic care approaches.

Mental Health Training & Advice:
- Deliver informal, tailored mental health training for Primary Care Network staff, including offering specialist advice to enhance mental health understanding within the team.
About the Location
You’ll be embedded within General Practice locations as part of a neighbourhood-based Primary Care Network, working closely with integrated teams at the Living Well Centres in Lambeth. This collaborative setup ensures a seamless, patient-centred approach to care.
Requirements
