About the Role

Are you passionate about enabling people to live independently with greater choice and control over their lives? We are seeking a dedicated and compassionate Personalisation Officer (Self-Directed Support Practitioner) to join our Commissioning Market Development team.

This exciting opportunity supports individuals receiving direct payments, helping them access the care and support they need to achieve the outcomes most important to them. Working closely within a collaborative, person-centred, and inclusive service, this role offers the chance to develop specialist knowledge in:

  • Direct payments and personal budgets
  • Community-based care approaches
  • Person-centred planning and support

Key Objectives

  • Support people with care and support needs (older people, physical disabilities, learning disabilities, mental health needs, and disabled children) to set up and manage direct payments.
  • Work with individuals and their families to identify and access the right support options, ensuring they have the skills, independence, and agency to manage their own budgets.
  • Cost direct care packages accurately to ensure allocated budgets are adequate for agreed support plans.
  • Collaborate across adult social care, children’s social care, and commissioning teams, enhancing cross-service partnerships.
  • Actively promote the Personal Assistant (PA) role in Barnsley, supporting recruitment and awareness initiatives.
  • Provide joint and lone working across the borough, requiring both collaborative teamwork and independent decision-making.

Requirements & Expectations

Key Competencies

✔ Exceptional person-centred practice – You must put individuals at the heart of all decisions, demonstrating an unwavering commitment to their autonomy. ✔ Analytical and organisational skills – Cost-effective assessment of support packages, knowing how to allocate budgets appropriately. ✔ Strong communication – Work substantively with individuals, service providers, and care teams; foster collaboration across services. ✔ Adaptability & resilience – Able to balance lone and joint visits across Barnsley, independently delivering high-quality support while being a proactive member of the team. ✔ Promoter of choice and independence – You’ll help embed self-directed support as a first option for people requesting care.

Job-Specific Criteria

  • Able to speak English at an appropriate level (professional communication essential).
  • Experience working in adult/carers social care (preferably with direct payments, self-directed support, or community care).
  • Willingness to ensure Effective DBS Checks are met, including Support for Overseas applicants.
  • Flexible approach to working hours in a dynamic environment.
  • Enthusiasm for contributing to Barnsley’s thriving inclusive communities through direct empowerment strategies.

Other Considerations
