About The Post
Role: Social Worker (for our Preparation for Adulthood Team) Focus Areas:
- Supporting young people aged 14–25 with diverse needs to achieve independence and life opportunities
- Direct delivery of strengths-based assessments, holistic support planning, and case management across Children’s & Adult’s social care needs
- Creating a person-centred, strengths-based approach aligned with the PFA model

Key Responsibilities

Advocate and protect young people’s rights while applying key legislation effectively, including: • The Care Act 2014 (adult safeguarding) • The Children Act1989 (child protection) • The Mental Capacity Act legislation • Human Rights law

Collaborate with the PFA team (social work practitioners and team manager) to develop and embed the Preparation for Adulthood framework

Foster trusted professional relationships with young people and their families to build confidence, resilience, and independence

Work across b crateful boundaries between Children’s and Adults social care

Contribute to setting standards for the new PFA team through shared learning and shared practice
Essential Criteria
- Qualification: State Registered Social Worker (SWRB or equivalent)
- Legislation Competence: Expert understanding of Care Act 2014, Children Act 1989, Mental Capacity Act, and Human Rights principles
- Core Skills: Proven assessment and planning skills within children’s/early adulthood social care
- Values: Upholding strengths-based, person-centred social work
- Experience: Strong preference for experience or passion working with young adults (14–25) with diverse needs

Bonus Areas (Desirable)

- Exposure to the Section 25 transition commitments (restoration and upholding Care Act rights)
