Pharmacist - Homecare

Patient-Focused Clinical Pharmacist (Responsible Pharmacist) – Homecare

Your Role

We’re looking for a patient-focused and clinically driven Homecare Pharmacist to join our Pharmacy team based at Thorncliffe Park, Sheffield, as a Responsible Pharmacist.

Carrying professional accountability, you will ensure the safe, effective, and compliant delivery of pharmacy services for homecare patients. This includes leading a team, supervising dispensing operations, driving service development, and contributing to outstanding patient outcomes.

Location & Working Arrangements

✅ Onsite, Monday–Friday ✅ 37.5 hours per week ✅ Address: Thorncliffe Park, Sheffield, S35 2PW (M1 Junction 35A) ✅ Parking: Free on-site available


Key Responsibilities

  • Leadership & Compliance
    • Act as the Responsible Pharmacist for the registered pharmacy, ensuring service delivery aligns with regulatory requirements (GPhC, legislation, SOPs).
    • Set and enforce high standards of patient care through mentorship and professional oversight of the pharmacy team.

  • Clinical Excellence

    • Perform clinically accurate prescription checks, dispensing validations, and formulation assessments.
    • Provide expert pharmaceutical advice to patients, nurses, healthcare professionals, and colleagues—resolving clinical queries where necessary.
  • Quality & Improvement

    • Conduct audits, risk assessments, incident reviews, and corrective actions (CAPAs).
    • Ensure adherence to GPhC standards, pharmacy legislation, and quality systems.
    • Participate in an on-call rota to support homecare outside normal hours when necessary.

What We Need from You

  • Qualified PharmD (or equivalent) with the MPharm Degree.
  • Registered Pharmacist with the General Pharmaceutical Council (GPhC).
  • Proven experience as a post-registration pharmacist in a clinical or homecare setting.
  • Deep clinical knowledge and commitment to pharmacy governance & ethical compliance.
  • Attention to detail with a patient safety-first mindset.
  • Ability to collaborate with internal teams and external healthcare providers.
