Pharmacy Technician (Integrated Care Settings) – West London NHS Trust
About the Role
West London NHS Trust is inviting applications for a motivated and patient-focused Pharmacy Technician to join their Integrated Care Pharmacy Team based at Clayponds Hospital (Northwick Park).
This varying and rewarding role spans community hospital wards, community health services, and outpatient clinics, playing a crucial part in delivering **high-quality medicines management and medicines optimisation services. The successful candidate will work closely with pharmacists, nurses, and multidisciplinary teams to ensure patients receive safe, effective, and timely access to their medicines.

Key Responsibilities
- Medicines Management & Optimisation:
Provide patient-centred medicines services across Clayponds Hospital and community healthcare settings, including:
- Supporting medicines reconciliation
- Conducting patient counselling & education on medicines use
- Assisting with medicines supply and discharge planning

Ward & Community Support:
- Perform medicines management visits to wards and community services
- Contribute to stock management activities for hospital pharmacies and community pharmacies
- Join audits and quality improvement projects to enhance medication processes
- Participate in maintaining and building a responsive pharmacy service across NHS partners

Collaborative Care: Work effectively both autonomously and within multidisciplinary teams, ensuring seamless delivery of care aligned with Trust values.
Person Specification
Essential Criteria
- Valid GPhC registration as a Pharmacy Technician
- Excellent organisational and communication skills
- Capacity to work independently and collaboratively
- Strong attention to detail and adherence to medicines safety protocols
Desirable
- Accredited Checking Technician (ACT) status
- Relevant medicines management qualifications
- Prior experience in NHS community or hospital pharmacy contexts
Support will be provided for optional upskilling and advanced training to meet business needs.
