Cardiac and Pulmonary Rehabilitation Physiotherapist – South West Yorkshire Partnership NHS Trust

The successful candidate will join our community-based cardiac and pulmonary rehabilitation team, providing high-quality care and support for patients recovering from cardiac or respiratory conditions—both in homes and community venues across Barnsley.
The Opportunity
Our team delivers specialist physiotherapy and rehabilitation care, assisting patients in regaining strength, mobility, and confidence. Ideal candidates will bring the following:
- Enthusiasm, autonomosity, and a problem-solving approach to patient care
- Strong clinical and communication skills, with proficiency in assessment, treatment, and progressive patient monitoring
- Experience in cardiac, pulmonary, or community physiotherapy (preferred but not mandatory)
- Eligibility to drive (essential; reasonable adjustments can be arranged for disabilities-related limitations)
- Flexibility to work across multiple venues and potential shifts (including evenings/weekends)
Key Responsibilities
Working as part of a multi-disciplinary team, your role will involve:
Clinical Care
- Conduct clinical assessments, including exercise testing, for cardiac and pulmonary patients
- Develop personalised treatment plans for individual/group rehabilitation programmes
- Monitor, assess, and adjust treatment plans based on patient progress
- Manage caseloads effectively, including triaging referrals and prioritising tasks daily

Collaboration & Patient Advocacy
- Advise medical, nursing, and allied health professionals regarding patient treatment needs
- Ensure detailed, up-to-date patient records (electronic and clinical documentation)
- Promote health education, disease management, and preventative health measures to meet national targets
- Organise health-screening programmes alongside specialist nurses/physiotherapists
Professional Growth & Compliance
- Maintain HCPC registration and continuing professional development (CPD)
- Support supervision of clinical students and assistants
- Participate in continuous improvement initiatives (audits, research, and training)
- Implement risk assessments for patients and clinical environments
- Maintain equipment, aids, and patient devices safely per Trust standards
Communication
- Engage with patients, carers, and families across treatment programmes
- Collaborate with hospital/community healthcare teams, social services, and voluntary organisations
Person Specification
Essential Requirements
- Qualification in Physiotherapy (BSc graduated diploma or equivalent).
- Current HCPC Registration.
- Experience in community-based therapy settings (preferably with cardiac/respiratory patients).
- Multi-disciplinary team experience.
- Evidence of risk assessment, clinical reasoning, and equipment-proficiency skills.
- A current driving licence with access to a vehicle for work-related travel (open to adjustments due to disability).
- Commitment to CPD and maintaining professional files.
Desirable
- Membership of the Chartered Society of Physiotherapy (MCSP).
- Specialist experience in neurorehabilitation, neurology, or other therapeutic areas.
- Leadership/involvement in team-based quality projects or audits.

Salary & Benefits
This is a permanent, full-time (flexible arrangements available) role at Agenda for Change Band 5, with a starting salary range of £32,073 to £39,043. We offer:
