Job Overview
Betsi Cadwaladr University Health Board in North Wales is a dynamic and patient-focused organisation. The Health Board is responsible for the direct management of a number of GP Practices and are developing these practices to be gold standard exemplars, creating services that are better able to meet the needs of the local population, easier to access and navigate, and are a rewarding and satisfying place to work.
We are looking for an enthusiastic and highly motivated Practice Nurse who believes in a vision for high-quality clinical care and compassion within Primary Care, and who wants to engage in delivering this in St Mark's Dee View Surgery, Connah's Quay.
The successful applicant should already have experience of long-term conditions and working in Primary Care or General Practice.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
Main Duties of the Job
The post holder will perform tasks prescribed and delegated by the GPs and Senior Nurses, working unsupervised to Patient Group Directions, agreed protocols, within an evidence base, and within defined competencies.
The main duties of the role include (but are not limited to):
- Undertake chronic disease management clinics, including Asthma, COPD/Spirometry, Diabetes, and Chronic Heart Disease.
- Identify, and manage as appropriate; treatment plans for patients at risk of developing a long-term condition.
- Perform basic nursing tasks such as ear syringing, blood pressure monitoring, ECGs, etc.
- Assess and care for patients presenting with wounds including the removal of sutures and clips and leg ulcer management.
- Participate in vaccination and immunisation programmes; administering injections, ensuring safe vaccine storage, and implementing cold chain policy.
- Undertake health screening, surveillance, and new-patient screens.
- Take pathology samples from patients for analysis (blood, urine, etc).
- Assess problems presented opportunistically by patients, dealing with minor illness as well as in minor illness clinics.

Essential Criteria
- Competent in basic nursing duties such as wound management, ear care, vaccinations/immunisations, and venepuncture.
- Knowledge of needs of patients with long-term conditions.
- Aware of accountability of own role and other roles in a nurse-led service.
- Knowledge of patient group directions and associated policy.
- Ability to prioritise workload and work under pressure.
- Ability to work and adapt to a changing and demanding environment.
- Accurate keyboard skills.
Desirable Criteria
- An understanding of QoF and the GMS Contract.
Experience
Essential Criteria
- Previous GP Practice Nurse experience.
- Experience of working with GP Clinical IT Systems.
- Experience of working in accordance with the UKCC Code of Professional Conduct and The Scope of Professional Practice.
Desirable Criteria
- Some community or primary care nursing experience.
Qualifications
Essential Criteria
- Registered General Nurse. Active statutory registration.
- Recognised qualification to diploma level or above in areas of chronic disease management.
- Demonstrable commitment to professional development.
Desirable Criteria
- Ability to speak Welsh.
