Job Summary
We are looking for an enthusiastic and skilled Practice Nurse to join our rural GP practice, Much Birch Surgery.
Both full time or job share will be considered.
Closing date: 30/06/2026
Interview date: 15/07/2026
This is an opportunity to work in a well-established, patient-focused practice with a strong team ethos and a reputation for delivering high-quality care. While we benefit from being part of a larger organisation (Taurus Healthcare), we have retained the supportive culture and close working relationships that come with being a smaller practice.
Our team is made up of GPs, Advanced Nurse Practitioners, Practice Nurses, Healthcare Assistants, dispensary staff, and administrative colleagues who work closely together to provide the best possible care for our patients.
What makes our practice different is the emphasis we place on teamwork, support, and professional development. Clinical staff participate in regular group clinical supervision sessions, protected learning time, and daily team huddles. We actively encourage staff to contribute ideas and help shape how services are delivered and are always happy to look at different innovative approaches to care.

The Much Birch GMS contract was novated to Taurus Healthare in February 2024, but the practice team remains unchanged as does the identity of the surgery in the local community.
Much Birch is:
- A busy rural dispensing practice at the heart of the community.
- Located in the village of Much Birch, between Ross-on-Wye and Hereford city.
- Covering an area reaching from the edges of Hereford to Ross on Wye, and extending into the villages of the Golden Valley.
- List size of circa 5,000+ patients
- A supportive clinical team of GPs, ANPs, Paramedics, Practice Nurses, HCAs, Physiotherapist, Social Prescribers

Job Description
Job responsibilities
Our Practice Nurses provide high-quality nursing care to patients registered with the practice, promoting health, preventing disease, and supporting the diagnosis and management of acute and chronic conditions.
This role will work collaboratively with the multidisciplinary team to deliver safe, effective, and patient-centred care.
Key Responsibilities (according To Training And Experience)
- Undertake patient assessments and provide evidence-based nursing care.
- Deliver chronic disease management clinics
- Administer vaccinations and immunisations in accordance with national and local guidelines.
- Provide cervical screening (where appropriately trained).
- Contraception care and sexual health assessment and signposting
- Perform wound care, dressings, and suture removal.
- Conduct health promotion and lifestyle advice, including smoking cessation, weight management, and healthy living.
- Carry out NHS health checks and screening programmes.
- Undertake phlebotomy, ECGs, blood pressure monitoring, and other clinical procedures.
- Administer medications and injections under PGDs or PSDs as appropriate
- Provision, ordering and safe storage of vaccines for all ages in line with national guidance and an up to date knowledge of PGDs.
- Administering routine injections, as prescribed
- Recognise and respond appropriately to medical emergencies.
- Will play a vital role in providing, leading, coordinating and evaluating care that is compassionate, evidence based, and personalised.
- Are accountable for their own actions and those who they delegate to and must be able to work autonomously, or as an equal partner with a range of other professionals.
- Will provide nursing care for people who could have complex and concurrent mental, physical, cognitive, and behavioural care needs
- Will make an important contribution to the promotion of health, health protection and the prevention of ill health by empowering people to exercise choice, take control of their own health decisions and behaviours and by supporting people to manage their own care where possible.
- Simple and complex wound management, doppler and compression
- To act as a link worker for an identified population of patients for example those with an identified learning disability

- Work autonomously and collaboratively.
- Excellent communication skills both written and verbal.
- Competent use of Microsoft office and other IT programmes.
- Able to organise and plan own workload effectively.

Qualifications
Essential
- Part 1 of the Nursing and Midwifery Council (NMC) Register.
- NMC supervisor/assessor (expected for all registered nurses).
- Degree or equivalent.
- Able to demonstrate Continuing Professional Development (CPD) that is applicable to general practice nursing.
Desirable
- GPN fundamentals qualification or demonstrable equivalent training or experience.
