Our Primary Care teams at DHU Healthcare provide accessible, compassionate support around the clock, ensuring patients receive the right care at the right time. Working across GP practices, NHS 111, out-of-hours services, and community settings, our skilled clinicians, nursing staff, and support colleagues collaborate to deliver high-quality, patient-centred care. Together, we respond to a wide range of health needs while creating an environment where you're supported to grow, develop, and thrive in your career.
As a Clinical Pharmacist you will:
- Deliver expert clinical medicines management, including structured medication reviews, medicines reconciliation, and support for patients with complex polypharmacy.
- Work as part of a multidisciplinary team across the practice and Primary Care Network to manage long-term conditions, improve health outcomes, and reduce health inequalities.
- Lead on safe, effective prescribing systems, including repeat prescription processes and medicines optimisation across transitions of care.
- Contribute to quality improvement, clinical audit, and the Quality and Outcomes Framework, driving service efficiency and high standards of patient care.
- Provide specialist pharmaceutical advice, education, and independent prescribing support, promoting best practice and alignment with local and national healthcare priorities.
