Work Authorization: Must be based within the European Union (hired via Deel where applicable)
About The Role
We're looking for a Principal Software Engineer to design and build the core infrastructure powering autonomous AI clinicians.
This is a highly hands-on engineering role focused on building large-scale distributed systems, AI agents, secure healthcare infrastructure, and production-grade backend platforms. You'll work directly with founders, hospitals, and healthcare providers to solve complex technical problems that have real-world impact.

What You'll Do
AI Platform Engineering
- Architect and build AI agent systems for healthcare applications
- Design scalable distributed backend services
- Develop secure healthcare infrastructure handling sensitive clinical data
- Build low-latency communication systems supporting voice and video workflows
Production Engineering
- Build scalable Python services powering enterprise healthcare platforms
- Design APIs, orchestration systems, and backend infrastructure
- Optimize system performance, scalability, and reliability
- Improve deployment, monitoring, and operational excellence
AI Systems
- Build production systems around LLMs, AI agents, and reinforcement learning
- Design agent orchestration frameworks
- Integrate AI capabilities into real-world healthcare workflows
- Continuously improve system accuracy and operational performance
Technical Leadership
- Translate ambiguous product challenges into scalable engineering solutions
- Partner directly with founders and product teams
- Help shape technical architecture and engineering standards
- Mentor engineers through technical leadership and example

What We're Looking For
Must Have
- 5+ years building complex production software systems
- Strong Python engineering experience
- Experience designing scalable distributed systems
- Track record of building production-grade backend platforms
- Experience turning ambiguous requirements into reliable solutions
- Strong system design and architecture skills
- Startup or high-growth company experience
- High ownership and strong problem-solving mindset
- Excellent communication skills
- Based in Europe
Nice to Have
- Healthcare technology experience
- AI, LLM, or Agentic AI experience
- Reinforcement Learning
- Cloud infrastructure (AWS, GCP, Azure)
- Enterprise software experience
- Open-source contributions
- Background in Mathematics, Computer Science, or Physics
- Experience building products from zero to production
