Project Manager – Adult Social Care | Hackney Council

Key Requirements for Successful Candidates

We’re looking for instinctive, strategic project leaders with:

Core Skills & Experience

✅ Deep knowledge of adult social care—with a proven track record in project management, change leadership, and transformation resuscitation in local authority or parallel settings. ✅ Networking & strategic relationships—experience building alliances across diverse stakeholders. ✅ Leadership in complex, evolving environments—implementing large-scale change while retaining focus on result-driven impact. ✅ Potential for line management—if selected for these roles, you may manage Project Officers or Management Trainees.

Available Roles

1. Project Manager – Understanding Demand & Maximising Prevention

  • Contract type: Full-time, Fixed Term Contract (FTC) / Secondment for 2 years
  • Focus: Prevention in Adult Social Care transformation
  • Scope of work:
    • Leading a prevention programme across the health and social care continuum
    • Implementing initiatives that:
      • Redesign holistic support when individuals first enter social care
      • Stabilise individuals during delays in assessment/review processes
      • Support those with complex needs to prevent further dependency
    • Cultural transformation: Embedding a preventative mindset organisationally, so staff daily build capability to prevent, reduce, and delay support needs.

2. Project Manager – Reablement (x2 roles)

  • Contract type: Full-time, Fixed Term Contract (FTC) / Secondment for 1 year
  • Focus: Piloting and expanding a reablement and enablement-first approach
  • Scope of work:
    • Previously piloted with hospital discharge referrals
    • Next phase: Scaling up to include community-based referrals
    • Requires significant process optimisation and alignment with evidence-based practice
