Permanent (3 Vacancies) Full-time (37 hours per week) Hybrid – Welbeck House, Sherwood Energy Village (NG22 9FF) & flexible across North Nottinghamshire Salary: £25,583 – £26,403 per annum

About the Role and Organisation

The Maximising Independence Service (MIS) focuses on early intervention, prevention, and promoting independence and wellbeing for individuals across Nottinghamshire. Our goal is to empower people by providing information, advice, and goal-focused support to help them live as independently as possible.

This service supports through initiatives such as:

  • Enablement & Reablement
  • Benefits advice
  • Signposting to community resources
  • Equipment & minor home adaptations
  • Carers support & Single Handed Care reviews
  • Blue badge eligibility assessments
  • Collaborations with i-work and other third-party providers

You would join a multidisciplinary team, working under the direction of the Enablement Manager, to deliver personalised, strengths-based support across Newark, Sherwood, and Bassetlaw. Each role will focus on helping individuals (aged 17+) to achieve meaningful goals for up to 12 weeks, such as improving daily living skills, accessing community activities, learning new abilities (e.g., cooking, public transport), or navigating social support.

Key Responsibilities

  • Provide person-centred independence support tailored to individual needs, goals, and preferences.
  • Develop and implement personalised action plans while working within a choice and control framework.
  • Conduct reinablement and moving/handling assessments where necessary, ensuring safety and dignity are upheld.
  • Deliver enabling training and practical activities (e.g., mobility, housekeeping, finances) to individuals across all areas effectively.
  • Collaborate with carers, healthcare professionals, and community services to coordinate holistic support.
  • Deliver novel and effective methods of teaching, such as community-based projects or vocational capacity building.
  • Support the Blue Badge scheme process in partnership with other teams.
  • Uphold safeguarding duties for vulnerable adults and young people (where applicable).
  • Regularly review progress and adjust support plans to maximise independence gains.
  • Engage in continuity planning and handovers with colleagues to ensure ongoing services.
  • Use digital systems and tools (e.g., case management, provision mapping) to record and monitor progress.
  • Represent the service professionally at external partnership meetings as required.

Essential Criteria

  • Experience in social care or community support roles (care coordination, enabling services, voluntary work, etc.).
  • Demonstrates understanding of strengths-based approaches in independent living support.
  • Evidence of flexibility, reliability, and time management skills with a proactive approach.
  • Committed to professionalism, open communication, and ethical standards at all times.
  • Commitment to adhering to safeguarding practices and whistleblowing policies.

Desirable Criteria

  • Towpath Navigator or similar referral scheme skills.
  • Reablement or community support qualifications (level 3 aged care, enablement diploma, etc.).
  • Previous experience with dementia or disability advocacy.
  • Qualifications or training in mentorship or career coaching.
  • Competency with person-centred planning or Carers’ Assessments.
