About the Role

The Maximising Independence Service (MIS) is dedicated to early resolution, prevention, and promoting independence and wellbeing. Its aim is to support individuals in maximising their independence through:

  • Information, advice, and guidance
  • Setting and achieving short-term goals
  • Identifying tailored support

Key Areas of Support

  • Enablement
  • Reablement
  • Benefits advice
  • Signposting to community support
  • Equipment and minor adaptations
  • Carers support and advice
  • Support from i-work
  • Assessment for Blue Badge eligibility eligibility

In collaboration with other providers, the team works to enhance individual independence and quality of life.

Your work will involve:

  • Supporting individuals aged 17.5+ across diverse service areas
  • Setting and delivering outcomes within 12-week timeframes
  • Focusing on goals meaningful to individuals—including public transport navigation, social engagement, or skills building

The role is highly varied,Teams aligned with a multidisciplinary framework for collaborative problem-solving and impacted by expert supervision collaborate closely alongside Enablement Managers, ensuring truly optimised customer success.

The Team

Post Type: One full-time position within the Mid Notts team (covering areas including Mansfield and Ashfield) Key collaboration opportunity exists between frontline colleagues and the Enablement Manager amongst a wider multidisciplinary team.


Roles & Responsibilities

Target Population

  • Individuals aged 17.5+, regardless of health status.
  • Holistic strategy based on strengths-based approach.

Working Flexibly

Flexible work patterns required to accommodate the dynamic needs of different communities.

Essential Criteria: Un leading task/more specific operational capabilities and collaborative working partnerships necessities

Location and Travel Requirements

Due to the nature of the role and areas serviced, securing a driving licence and access to a private vehicle is mandatory if possible. A reasonable discretion applies to applicants whose disability prevents it.* Modes of Work: Primarily addressing community settings and individual residences monthly.

  • Team Approach Works with supportive colleagues in disablement managers setting to resteam collaborators areas to achieve best outcomes
