Job Description

Contract Details

This is a fixed term contract for up to 12 months. The hours are part time, 30 hours a week.

About the Role

The Health Visiting Service is part of the 0-19 Public Health Nursing Service, providing health information, advice, support and interventions based on the identified needs of a child, their family, and their communities. This is often done in partnership with other professionals and agencies who work with or interact with children to ensure the best outcomes are achieved for the child.

The specific job purpose is to support the Public Health Nursing Team within the 0-5 Health Visiting Service with a particular focus on supporting families with children with additional needs.

The Public Health Nurse is a Practitioner working with children and families within the community, working in an effective, safe, and inter-professional manner with all agencies.

The role includes supporting families and children with special educational needs and disabilities (SEND) and supporting their progression on the Gloucestershire Health and Care NHS Foundation Trust (GHC) policies such as the SEND Graduated Pathway for Gloucestershire, SCAAS pathway, Safeguarding, promotion of health, prevention of ill health, and protection of children, which are key priorities. This is undertaken in partnership with other agencies.

The Public Health Nurse works under the instruction and supervision of a SCPHN – Health Visitor in accordance with GHC protocols and competencies.
