Liquid Personnel is currently recruiting for an experienced Social Worker

Location

Civic Centre in Newport

Responsibilities

As a Social Worker within the Learning Disabilities Team, you will:

  • Undertake assessments and reviews for adults with learning disabilities
  • Manage a varied caseload while delivering strengths-based, person-centred interventions
  • Work collaboratively with service users, carers, health professionals, and partner agencies
  • Support individuals across front door services, community teams, and hospital discharge pathways
  • Ensure that care and support plans are tailored to meet individual needs
  • Assess and manage complex risks
  • Contribute to safeguarding processes
  • Work within relevant legislation, including the Mental Capacity Act 2005
  • Maintain accurate records and reports
  • Play a key role in ensuring high standards of practice

Qualifications and Experience

To be successful in this role, you must have:

  • Qualified Social Worker status
  • Social Care Wales registration
  • Experience assessing and reviewing adults with learning disabilities
  • Strong understanding of strengths-based and person-centred practice
  • Experience working within front door, community, or hospital discharge services
  • Knowledge of the Mental Capacity Act 2005 and adult safeguarding legislation
  • Experience managing complex cases and risk
  • Excellent written and verbal communication skills
  • Strong analytical and decision-making abilities
  • Experience using Microsoft Office and electronic recording systems
  • Understanding of confidentiality and professional standards
  • Commitment to continuous professional development
  • A genuine passion for achieving positive outcomes for individuals and carers
