Responsibilities
As a Social Worker within the Learning Disabilities Team, you will:
- Undertake assessments and reviews for adults with learning disabilities
- Manage a varied caseload while delivering strengths-based, person-centred interventions
- Work collaboratively with service users, carers, health professionals, and partner agencies
- Support individuals across front door services, community teams, and hospital discharge pathways
- Ensure that care and support plans are tailored to meet individual needs
- Assess and manage complex risks
- Contribute to safeguarding processes
- Work within relevant legislation, including the Mental Capacity Act 2005
- Maintain accurate records and reports
- Play a key role in ensuring high standards of practice

Qualifications and Experience
To be successful in this role, you must have:
- Qualified Social Worker status
- Social Care Wales registration
- Experience assessing and reviewing adults with learning disabilities
- Strong understanding of strengths-based and person-centred practice
- Experience working within front door, community, or hospital discharge services
- Knowledge of the Mental Capacity Act 2005 and adult safeguarding legislation
- Experience managing complex cases and risk
- Excellent written and verbal communication skills
- Strong analytical and decision-making abilities
- Experience using Microsoft Office and electronic recording systems
- Understanding of confidentiality and professional standards
- Commitment to continuous professional development
- A genuine passion for achieving positive outcomes for individuals and carers
