Adult’s Social Worker Opportunity in Conwy
Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s team, based in Conwy.
Responsibilities

Hospital discharge planning
- Lead safe and timely discharges by completing strengths-based assessments, coordinating MDT input, and arranging appropriate community or residential support in line with the Social Services and Well-being (Wales) Act 2014.

Adult safeguarding
- Identify, investigate, and manage safeguarding concerns for older adults and vulnerable adults, undertaking Section 126 enquiries, risk assessments, and protection planning with partner agencies.

Complex case management
- Hold a varied caseload including hospital-based referrals, cognitive decline, physical frailty, and long-term conditions; complete Care and Support Plans, Mental Capacity Act assessments, and best-interest decisions.

Qualifications and Experience
- To be successful in this role you must have:
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
