Quality Improvement & Service Transformation Facilitator

Quality Improvement & Service Transformation – Project Support Role

Temporary or Permanent (as per needs) – Hywel Dda University Health Board

We have an exciting opportunity within our Quality Improvement & Service Transformation Team and are seeking individuals with experience of working within continuous quality improvement.

You will work as part of a dynamic team across the four acute sites at Hywel Dda on specific projects identified by the Quality Improvement & Service Transformation Team.


About the Role

To assist in the development and delivery of improvement and transformational change projects, with the aim of achieving high-quality care.


Job Summary / Purpose

Your core responsibility is to support the provision of the highest quality patient care through personal actions and continuous improvement.

Key responsibilities include:

  • Project Planning & Execution

    • Help develop project/improvement plans and support their implementation across the Health Board.
    • Lead the testing of changes using the PDSA (Plan-Do-Study-Act) methodology.
    • Demonstrate measurable impact of changes tested, clearly communicating findings.
    • Ensure specific projects assigned meet key milestones and deadlines, often under pressure.
  • Change Management

    • Guide teams through implementation of new ways of working, demonstrating an understanding of change processes.
    • Lead, motivate, and engage staff involved (stakeholder inclusion is critical).
    • Facilitate regular communication, feedback, meetings (with minute-keeping), and the production of approved reports.

  • Methodology & Reporting

    • Identify and assess project backgrounds using evidence-based improvement strategies.
    • Provide robust improvement methodologies to support teams in delivering projects.
  • Autonomous Work & Coordination

    • Prioritise workload effectively to meet deadlines in fast-paced environments.
    • Work closely with clinical and managerial staff, ensuring ongoing collaboration and cooperation.

Requirements

  • Experience in continuous quality improvement (CQI), Lean, or other change methodologies.

  • Strong project management skills and the ability to adapt workflows.

  • Demonstrated capability in data collection, analysis, and reporting.

  • Excellent communication and engagement skills for working with diverse teams.

  • Organisational skills for managing multiple tasks under tight deadlines.

  • Welsh language skills are welcome but not mandatory; applicants must speak in English.
