Salary: Band: 7, £32,061 – £36,363 per annum inc. 17% shift allowance, including early, late and weekend shifts.
37 Hours Per Week
You are required to work on a rota basis covering Monday – Sunday as per needs of the service. You are based in the office and will be completing community visits.
Your new role
The successful candidate will work in accordance with a rehabilitation philosophy that promotes maximum independence for all service users, supporting them to achieve their goals and regain confidence in daily living. They will welcome service users returning home from hospital, inpatient units or the community, completing initial assessments and ensuring a smooth transition with continuity of care.
The role involves assessing and monitoring service users’ progress, regularly reviewing and updating care plans in partnership with the multidisciplinary team and with the active involvement of the individual. The postholder will support service users with onward care arrangements, making referrals to Adult Social Care where appropriate and maximising the use of community resources to meet identified needs.

A key aspect of the role is providing practical support with a range of daily living activities, including mobility, transfers, personal care, household tasks, meal preparation, social engagement and access to community services and activities. The successful candidate will apply safe moving and handling practices at all times, ensuring the wellbeing and safety of both service users and colleagues. You will maintain accurate, timely and professional records, and supervise, support and monitor the performance of Reablement Support Assistants.
In the absence of the Case Coordinator, the successful candidate will provide operational support and guidance to the team, escalating concerns and helping to ensure the smooth running of the service. They will work both independently and collaboratively, identifying and escalating risks appropriately to maintain high standards of care, safety and service delivery.

Who Is The Person Summary
With experience of working with adults with care and support needs, you will have knowledge of the relevant statutory framework applicable to social care including Care Act 2014 etc. You will have experience of undertaking personal care and have a clear understanding of the impact disability can have on an individual’s daily life. You will be able to record, report and escalate information and maintain confidentiality. As well as a driving licence, you will have excellent communication skills and the ability to work flexibly to adjust to the needs of the service. A driving licence and access to a vehicle for work purposes as you will be required to make visits within the community
