About the Role
Do you enjoy working in the community as part of a multidisciplinary team helping people regain independence after illness? If promoting self-sufficiency through personalised care aligns with your skills, this is the role for you.
Norfolk First Support delivers reablement-based services to adults across Norfolk in their own homes for up to 6 weeks—helping clients regain essential skills, confidence, and overall independence after discharge from hospital or life-changing events.
Your responsibilities include:
- Collaborating in a multidisciplinary assessment team underpinned by Social Care standards
- Conducting home visits in Norwich and surrounding areas as part of a supportive reablement-recovery programme
- Assessing daily living needs, designing personalised plans to rebuild independence, and completing Care Act Assessments where additional support is required
- Utilising your judgement, initiative, and professionalism in developing client-centred solutions
Responsibilities
Core Duties
- Contribute to a team-based rehabilitation focus, ensuring client accessibility in their home environment
- Assess and plan to remove barriers to independence, tailoring interventions to meet specific needs
- Support individuals at critical transitional stages (e.g., post-hospital discharge, illness recovery)
- Act within legal and policy frameworks of Health and Social Care while maintaining clarity and ethics in interactions
- Provide ongoing care reviews and advocate for sustainable living solutions with constructive outcomes
- Asset current organisational documentation and reflect best practice to support your team’s effectiveness

Practical Employment Details
- Location: The Hive, Norwich Community Hospital, with fieldwork across homes in the area
- Hours and Flexibility: Core hours Monday–Friday, with potential introduction of flexible (rota) weekend availability based on operational needs
Requirements
To apply for the Reablement Assistant Practitioner position, we seek candidates who can demonstrate:
Qualifications and Experience
- Relevant practical experience in community-based or supportive roles
- A keen understanding of person-centred care, especially in re-enabling independence
- Familiarity with current Health & Social Care policies and legislation
Skills and Attributes
- Strong foundations in problem-solving, multi-tasking, and adaptability
- Clear communication with diverse populations—including service users, associates, and external professionals
- Confident navigation of judgement-heavy environments, with an ability to act decisively in professional settings
- Note: This role is not eligible for Visa sponsorship
