37 hours per week | £32,061 to £33,699 per annum | Grade G

Location: The Hive, Norwich Community Hospital, Norwich


Would you like to work in the community as part of our team of Occupational Therapists and Support Workers, creating person-centred personal plans that help people set and achieve their goals following an illness?

Role Overview

We have opportunities available to visit people in their own homes in Norwich and surrounding areas.

Norfolk First Support delivers services to adults across Norfolk in their own homes for up to 6 weeks, assisting them to regain skills they may have lost following an illness or hospital admission, enabling them to regain their independence while also reviewing any ongoing support needed.

In this role, Resembling Assistant Practitioner, you will:

  • Be part of a multidisciplinary assessment team.

  • Work alongside other Social Care professionals to support people who may have recently been discharged from hospital or where they have a change of circumstances requiring home support.

  • Assess and review daily living needs.

  • Develop personalised plans to help individuals regain skills and confidence to remain independent in their home.

  • Complete a Care Act Assessment if ongoing services are required.

All training will be provided to ensure competence and confidence in delivering this critical role.


Responsibilities

  • Assessment & Planning:
    • Evaluate and monitor daily living needs.
    • Create tailored plans to facilitate skill recovery and confidence-building for independence.
    • Conduct Care Act Assessments (if required).

Working Pattern:

  • Current contract hours are Monday to Friday.

Key Requirements

Applicants must demonstrate:

  • Experience: Working with people in a Social Care or similar context.
  • Communication: Ability to engage clearly and empathetically with both clients and professionals.
  • Policy Knowledge: Understanding of current Health and Social Care policies, practices, and legislation.
  • Situational Awareness: Capacity to assess environments, make considered judgements, and adapt approaches swiftly.
