We’re looking for a Reablement Planning and Coordination Hub Manager to lead the operational heart of our Vale Community Resource Service (VCRS). About The Role

This is a dynamic leadership role where no two days are the same. You’ll oversee the day-to-day running of the Reablement Hub, ensuring services are delivered safely, efficiently, and in line with regulatory requirements.

You will:

  • Lead and manage a team of Reablement Coordinators, supporting performance, wellbeing, and development
  • Oversee capacity, demand, and staff deployment, ensuring resources are used effectively
  • Coordinate care packages, reviewing and tailoring support in collaboration with health professionals
  • Work closely with hospital discharge teams and partners to keep patient flow moving and avoid unnecessary admissions
  • Use data and insight to monitor performance, improve processes, and maintain compliance
  • Provide supervision, coaching, and leadership to drive a high-performing team culture

About You

We’re looking for a confident, compassionate leader who can balance operational demands with a person-centred approach.

You will have:

  • Management experience in a social care setting
  • Strong experience managing workloads, rotas, and service performance
  • Excellent leadership, communication, and decision-making skills
  • Knowledge of key legislation including the Social Services and Wellbeing (Wales) Act 2014
  • Relevant Health and Social Care qualifications and Social Care Wales registration
  • A flexible, proactive approach with a commitment to continuous improvement

Additional Requirements

  • Ability to travel across the Vale of Glamorgan as required (car user)
  • Willingness to work flexibly, including evenings and weekends
  • Commitment to equality, Welsh language standards and continuous development
  • Ability to contribute to training and staff development

This role involves working with vulnerable adults and requires a Disclosure and Barring Service (DBS) check (Enhanced) as part of the recruitment process.
