Reconnect Navigator – Reconnected to Health

About the Role

Location: HMP Northumberland, Wansbeck Rd, Morpeth, NE65 9XG

Working Hours: 37 hours per week, Monday to Friday (08:00–16:00)

Contract Type: Permanent

Salary: £25,877.80 – £34,240 Starting salary depends on experience and progression within the band.

About the Role – Reconnected to Health

Waythrough partners with Spectrum Community Health CIC and Tees, Esk and Wear Valleys NHS Trust to deliver recovery services across all seven North East prison establishments.

Within the prison healthcare system, we provide:

  • Recovery coordination
  • Psychosocial interventions
  • Continuity of care planning

We work both in prisons and within community mental health services, fostering strong links with North East drug and alcohol support networks.

As a Reconnect Navigator, you will help individuals preparing to leave prison overcome health-related barriers, build positive community connections, and access the necessary support for a sustainable recovery.


Key Responsibilities

  • Manage a caseload of service users preparing for release from prison
  • Develop person-centred recovery plans
  • Support access to healthcare, recovery, and community services
  • Collaborate with prisons, probation services, healthcare teams, and community organisations
  • Promote continuity of care and positive recovery outcomes
  • Maintain accurate records and comply with prison and organisational policies
  • Contribute to a resilient, recovery-focused working environment

Requirements

  • UK driving licence (must be valid)
  • Own transport

Essential Qualifications And Skills

  • Preferred: NVQ Level 3 in Health and Social Care or equivalent (or commitment to working towards)
  • Proficiency in Microsoft Office
  • Ability to produce assessments, recovery plans, and risk management plans

Knowledge and Experience

  • Understanding of substance misuse interventions (MI, PSIs, CBT, ITEP, brief interventions)
  • Familiarity with recovery models and partners who support people in this field
  • Experience supporting individuals with complex needs
  • Knowledge of safeguarding and risk management approaches
  • Ability to work collateral with partner agencies and stakeholders
