Reconnect Navigator – Reconnected to Health

Working Hours

37 hours per week (Monday to Friday, 08:00–16:00)

Contract & Salary

Contract type: Permanent Salary: £25,877.80 – £34,240 Note: The listed salary reflects full earning potential. Starting salaries depend on experience and progression within the band.

About the Role

We are proud to be part of Reconnected to Health, delivering drug and alcohol recovery services across all seven North East prison establishments in partnership with:

  • Spectrum Community Health CIC
  • Tees, Esk and Wear Valleys NHS Trust

Within the prison healthcare system, we provide:

  • Recovery coordination
  • Psychosocial interventions
  • Continuity of care planning

Working closely with community services nationwide, we help men and women in custody adapt, recover, and reintegrate successfully.

As a Reconnect Navigator™, you will:

  • Support individuals preparing to leave prison by helping them overcome health-related barriers
  • Build positive community connections
  • Ensure access to the support needed for recovery and wellbeing

Key Responsibilities

  • Manage a caseload of service users preparing for release from prison
  • Develop person-centred recovery plans
  • Support access to healthcare, recovery, and community services
  • Work collaboratively with prisons, probation, healthcare teams, and community organisations
  • Promote continuity of care and positive recovery outcomes
  • Maintain accurate records and follow organisational/prison procedures
  • Contribute to a positive and recovery-focused environment

Requirements

Access & Licences

  • Full valid UK driving licence
  • Access to own vehicle (essential for this role)

Essential Qualifications & Skills

  • NVQ Level 3 in Health and Social Care (or equivalent, or willingness to work towards)
  • Proficiency in Microsoft Office programmes
  • Ability to complete assessments, recovery plans, and risk management plans

Knowledge & Experience

  • Understanding of interventions including:
    • MI (Motivational Interviewing)
    • PSIs (Psychosocial Interventions)
    • CBT (Cognitive Behavioural Therapy)
    • ITEP (Integrated Treatment Effectiveness Programme)
    • Brief Interventions (for substance misuse)
  • Knowledge of substance misuse, recovery models, and recovery planning
  • Experience supporting individuals with complex needs
  • Familiarity with safeguarding and risk management
  • Ability to work effectively with partner agencies and stakeholders
