Working Hours: 37 hours per week (Full Time)

Contract Type: Permanent (Full Time)

Salary: £25,877.80 to £34,240.03 per annum

About the Role

Forward Leeds is the city's alcohol and drug service, providing advice, support and treatment for adults, young people and families. We're looking for a Recovery Coordinator to join our Shared Care and Homelessness Team.

This specialist role supports adults who are prescribed opiate substitution treatment through our Shared Care pathway, alongside individuals experiencing homelessness or facing multiple and complex needs.

Working closely with GPs, primary care clinicians and partner organisations, you'll coordinate person-centred recovery plans, provide ongoing key working and psychosocial interventions, and help people overcome barriers to treatment and recovery. You'll play a vital role in improving health outcomes, reducing substance-related harm and supporting individuals to achieve their recovery goals.

This role is split across two locations, working two days each week from Forward Leeds, 74 Kirkgate, Leeds, LS2 7DJ, and three days each week from the Shared Care Health Practice on York Street, Leeds, working alongside primary care colleagues to deliver integrated support.

What You'll Do

As a Recovery Coordinator, you will:

  • Manage a caseload of adults prescribed opiate substitution treatment, including people experiencing homelessness and multiple disadvantage.
  • Complete comprehensive assessments, recovery planning and regular reviews to support individuals throughout their recovery journey.
  • Deliver structured one-to-one psychosocial interventions and recovery-focused key working sessions.
  • Work closely with GPs, nurses and the multidisciplinary team at the Shared Care Health Practice to coordinate treatment and ongoing support.
  • Develop strong partnerships with housing providers, health services and community organisations to improve outcomes for service users.
  • Deliver harm reduction interventions and promote safer drug use practices, including BBV awareness, naloxone, needle exchange and other evidence-based interventions where appropriate.
  • Complete safeguarding, risk assessments and risk management plans, ensuring the safety and wellbeing of service users.
  • Maintain accurate and confidential electronic case records and contribute to service performance and quality standards.
  • Participate in multidisciplinary meetings, supervision, training and continuing professional development.

Qualifications

  • NVQ Level 3 in Health & Social Care (or equivalent), or a willingness to work towards this qualification.
  • A professional qualification in addiction studies, counselling, social work or a related discipline is desirable.

Experience

  • Experience of working with adults affected by substance misuse, particularly opiate dependence, and an understanding of the challenges associated with homelessness, health inequalities and multiple disadvantage.
  • Recovery Practice: Knowledge of recovery-focused practice, harm reduction, safeguarding, risk assessment and person-centred care planning.
  • Partnership Working: Experience of working collaboratively with healthcare professionals and partner agencies to deliver coordinated, holistic support.
  • Communication Skills: Excellent interpersonal and communication skills, with the ability to engage, motivate and build positive relationships with people facing complex challenges.
  • IT Skills: Confident using Microsoft Office and electronic case management systems to maintain accurate records.
  • Teamwork & Initiative: Able to work confidently across different sites while contributing positively to a supportive multidisciplinary team.
