Creative Support are recruiting an experienced Support Worker to join our mental health team in Gateshead, working with individuals over 18 who have homelessness-related needs or are at risk of homelessness.
About the Role
You will be part of an outcome-focused floating support team providing time-limited assistance to people requiring housing-related support. Commissioned by Gateshead Council, the service operates from an office in Gateshead, supporting clients through:
- Short- and long-term interventions tailored to each individual’s needs, including:
- One-to-one support in home settings
- Drop-in sessions and group-based activities
- Signposting to essential services to ensure stable tenancies and community integration
Your responsibilities will include holistic case management, promoting independence and improving wellbeing while reducing systemic instability. This includes developing tailored plans for tenancy resilience, financial sustainability, and overall quality of life.
Key Responsibilities
You will work with individuals requiring extensive support, using a multi-agency, recovery-focused approach. Core activities include:
- Tenancy and housing stability – Supporting resettlement, preventing homelessness, and sustaining tenancies.
- Financial management – Assisting with income maximisation and budgeting for long-term self-sustainability.
- Health and wellbeing – Helping with smoking cessation, alcohol/substance reduction, diet, exercise, and mental health management.
- Employment and meaningful activities – Connecting clients to volunteering, training, and job opportunities.
- Probation and justice support – Collaborating with criminal justice agencies to assist rehabilitation.
- Community integration – Encouraging engagement with local amenities, leisure, and civic life.

Strategic Objectives
- Develop exit strategies that foster resilience and independence.
- Contribute to clients’ self-reported improvements in quality of life, confidence, and local connection.
- Work flexibly on a rota basis (may include evenings, weekends, or night shifts) while fostering strong community ties.
Requirements
- Experience in supporting individuals with complex behavioural or mental health needs, particularly those experiencing homelessness.
- Ability to develop and deliver holistic support plans based on individual strengths and barriers.
- Strong interpersonal and advocacy skills with a commitment to inclusion, anti-racism, and human rights.
- Proficiency using case management software (familiarity with frameworks like Outcome Star would be helpful).
- Ability to work autonomously but as part of a collaborative, multi-disciplinary team.
- UK right to work – applicants must be eligible forاریض in the UK.
Desired Qualifications
- Interest in pursuing a QCF Diploma in Health and Social Care (training provided).
- Qualifications beneficial though not requirements: Level 3 Social Care, Counselling, or related qualifications (preferred).
