Role Overview
Our agency is currently seeking to engage a qualified Intake / Scheduling Coordinator to support our home health operations. We are looking for a highly efficient, detail-oriented professional with strong experience in end-to-end intake coordination and home health workflow management. This role is critical to ensuring timely patient admission, accurate documentation, and efficient clinician scheduling.

Key Responsibilities
- Processing and entering referrals into Axxess EMR
- Scheduling clinicians appropriately based on discipline, availability, and payer requirements
- Obtaining and verifying home health insurance authorizations
- Reviewing and processing physician orders
- Faxing and following up on orders with physicians
- Uploading and maintaining documentation and orders in Axxess
- Managing intake workflow from referral through admission and scheduling
- Understanding Medicare home health guidelines, including OASIS requirements and compliance expectations
- Working efficiently in a fast-paced, high-volume environment with minimal supervision
Qualifications
- Highly organized and detail-oriented
- Experienced in home health intake or coordination roles
- Able to work independently with strong follow-through
- Comfortable managing multiple priorities simultaneously
- Familiar with Medicare Conditions of Participation and home health compliance workflows
- Strong in communication with clinicians, physicians, and internal staff
