Role Overview

Our agency is currently seeking to engage a qualified Intake / Scheduling Coordinator to support our home health operations. We are looking for a highly efficient, detail-oriented professional with strong experience in end-to-end intake coordination and home health workflow management. This role is critical to ensuring timely patient admission, accurate documentation, and efficient clinician scheduling.

Key Responsibilities

  • Processing and entering referrals into Axxess EMR
  • Scheduling clinicians appropriately based on discipline, availability, and payer requirements
  • Obtaining and verifying home health insurance authorizations
  • Reviewing and processing physician orders
  • Faxing and following up on orders with physicians
  • Uploading and maintaining documentation and orders in Axxess
  • Managing intake workflow from referral through admission and scheduling
  • Understanding Medicare home health guidelines, including OASIS requirements and compliance expectations
  • Working efficiently in a fast-paced, high-volume environment with minimal supervision

Qualifications

  • Highly organized and detail-oriented
  • Experienced in home health intake or coordination roles
  • Able to work independently with strong follow-through
  • Comfortable managing multiple priorities simultaneously
  • Familiar with Medicare Conditions of Participation and home health compliance workflows
  • Strong in communication with clinicians, physicians, and internal staff
