Job Overview
We are seeking a motivated nurse to join our 20 bedded Intermediate Care Rehabilitation ward, caring for patients with frailty and complex needs including discharge to assess patients.
This is a fantastic opportunity for someone with experience in community or the the acute setting who is compassionate about delivering high quality patient centered care and clinical knowledge.
We are a vibrant busy and evolving team, bringing together a mix of experienced community nurses and nurses with acute backgrounds. We have onsite medical cover including twice weekly frailty consultant including a registrar.
We are a Nurse led unit with scope to to develop this further, actively promoting education, development and new ideas.

Main duties of the job
As a Registered Nurse On Our Ward You Will:
- Plan assess implement and evaluate individualised care in a timely manner ensuring high standards across all aspects of patient care and interventions are met.
- Deliver care that respects privacy dignity and individual religious and cultural beliefs
- Maintain and develop your knowledge, skills and clinical competence in line with NMC Code of professional conduct
- Ensure high standards of documentation, record keeping and reporting
- Uphold patient confidentiality at all times.
- Contribute to quality improvements on the ward and ongoing development of the service.
- Contribute to our learning environment, including teaching training and mentoring students and newly qualified staff.
- Promote and maintain a safe clinical environment adhering to infection prevention and health and safety standards.
- Communicating effectively with all staff and multidisciplinary team including patients, relatives and colleagues
- Support the management of ward resources and stock levels.
