Practice Nurse vacancy at Grove Village Medical Centre
Job Summary
We are working with Grove Village Medical Centre to recruit a Practice Nurse to join their friendly and supportive clinical team.
Available on a permanent or locum contract, this role offers flexibility. You’ll deliver high-quality nursing care in a busy GP surgery, covering areas such as:
- Chronic disease management
- Cervical screening
- Immunisations
- Wound care
- Health promotion
The practice is patient-focused and provides a modern, collaborative environment supporting staff development and making a meaningful impact on the local community.
Important Note: Due to clinical requirements, the successful candidate must have recent experience as a Practice Nurse in a UK GP surgery. Applications from candidates without at least some primary care experience will not be considered.
Main Duties of the Role
The Practice Nurse will provide high-quality nursing care within a GP surgery setting, including:
- Chronic disease clinics (e.g., diabetes, hypertension, asthma)
- Cervical cytology screening & health promotion
- Immunisations & vaccinations for adults and children
- Wound care (including ulcers & Doppler checks)
- NHS health checks & long-term condition support
- Travel medicine, pathology requests & result reporting
- Well-woman & well-man clinics
- Assistance with minor surgery under GP supervision
- Clinical record-keeping (adhering to NHS guidelines & legislation)
- Mental health assessment & support
- Emergency clinical deputising
- Safeguarding responsibilities & substance misuse referrals
Occasional additional tasks may arise based on workload, collaboration, or team needs.

Job Description
Core Responsibilities
The Practice Nurse will be responsible for the following key duties:
- Develop health promotion and wellbeing programmes
- Implement & evaluate individualised treatment plans for chronic disease patients
- Identify & support patients at risk of long-term conditions
- Provide routine nursing care (aligned with NICE, NSF & evidence-based practice)
- Deliver wound care services (including ulcers & Doppler checks)
- Offer travel medicine support
- Request & process pathology services & results
- Run chronic disease clinics, patient referrals & clinical records
- Ensure read codes accuracy and maintain chronic disease registers
- Lead well-women & well-men clinics
- Chaperone patients where required
- Aid GPs with minor surgery
- Prioritise health issues and address them appropriately & promptly
- Assist in clinical emergencies
- Recognise mental health needs and refer as required
- Manage vaccination programmes (adult & child)
- Support patient medication adherence
- Liaise with external health agencies (e.g., for vulnerable patients)
- Delegate clinically appropriate tasks safely
- Support safeguarding policies & substance misuse referrals
- Engage in opportunist health promotion activities
Secondary Responsibilities
Beyond core duties, the Practice Nurse will contribute to:
- Leading audit processes through structured cycles
- Monitoring medication safety (stock & disposal)
- Mentoring junior nursing staff
- Supporting local healthcare initiatives for improved patient care
- Shared learning activities within the practice
- Reviewing clinical practices aligned to NHS/UK policies
- Participating in significant event reviews and root cause analysis (RCA)

Person Specification
Clinical Knowledge
Essential
- Wound care (including suture/staple removal)
- ECG interpretation
- Venepuncture skills
- New patient medical assessments
- Chaperoning procedures
- Pathology requests & patient reporting
- Travel medicine advice
- Management of chronic diseases (e.g., diabetes, hypertension, asthma, COPD)
- Immunisation programmes (routine, childhood, travel-related)
- Women’s health (cervical cytology, contraception, 3Ps)
- Evidence-based practice awareness
- Clinical governance understanding
- Ability to record accurate clinical notes in accordance with legally required standards
- Awareness of record-keeping compliance (e.g., Patient Group DirectionsPGDs)
- Ability to recognise limits of scope of practice and refer appropriately to GPs
Desirable
- Knowledge of local public health priorities
- Familiarity with national/social determinants of health
Qualifications
