Registered Nurse – Home Care Team ("Revolution")

The Opportunity
"Bring Healing Into a Patient’s World"
As part of our "Revolution" team, you’ll:
- Design and oversee holistic care plans for homebound patients, managing post-injury recovery or chronic disease exacerbations.
- Conduct 5–6 community visits daily across Newton and its surrounding territories: Auburndale, Dover, Holliston, Natick, Needham, Sherborn, Waban, and Wellesley.
- Collaborate seamlessly with LPNs, therapists, social workers, and interdisciplinary teams to deliver patient-centred solutions.
- Provide clinical expertise across:
- Intravenous therapy, injections, tube feeding, and wound management
- Education (care plans, self-arrangement techniques)
- Documentation for electronic records (MGB Healthcare at Home)
Your role blends skilled nursing, care coordination, and emotional intelligence — with flexibility for varied geographic needs. Travel is a requirement within served areas.
Key Responsibilities
✔ Patient Care Leadership
- Respect the principles of community health standards, blending clinical acuity with research-backed best practices.
- Balance autonomy with interdisciplinary teamwork from admissions to discharge planning.
✔ Clinical Services
- Deliver intravenous drug administration, wound care, injections, and education tailored to patient needs.
- Voluntary management of compliance with skilled care requirements under state and insurer guidelines.
✔ Case Management
- Act as the full-circle overseeing agent for a caseload, advocating for holistic recovery.
- Build patient confidence through transparent care coordinator (with LPNs, therapists, and social workers).

✔ Documentation & Coordination
- Compile thorough electronic health records while balancing on-site visits with administrative duties.
- Engage in synchronous/electronic team huddles to streamline patient transitions.
✔ Client Advocacy
- Address and negotiate barriers to home care—within and beyond clinical scope, e.g., transportation, medical supply provision.
- Adhere to MGB re-admissions policies and Community Health Improvement Plan values during interaction.
Qualifications & Experience
Requirements
- Graduate from a Massachusetts-approved School of Nursing (or Regionally-recognised equivalent).
- Current unrestricted Massachusetts RN license (with disciplinary findings below thirty years).
- 1+ years nursing experience in acute care, preferred: prior home health or high acuity nursing.
- Valid driver’s license & reliable transportation to cover assigned service areas.
- Tech-ready: Experience with handheld medical devices or patient care software; proficiency in systems like EMR preferred.

- Prioritisation & Limits Management: Schedule 5-6 daily visits with charting/palliative updates while adhering to timely physician reporting.
