Band 5 Registered Nurse – Home First Team (Wrexham)

About the Role
An exciting opportunity has arisen for an enthusiastic and motivated Band 5 Registered Nurse to join the multi-disciplinary Home First Team based in Wrexham.
The postholder will be responsible for:
- Ongoing screening of patients
- Identifying care availability to facilitate safe and timely discharge from hospital
- Providing ongoing support for ongoing or complex needs in the community
- Implementing the Discharge to Assess model of care
- Promoting the Home First ethos through therapeutic interventions in patients' own environments and on Ready for Home wards
The role operates 37.5 hours per week, covering 7 days (Monday–Sunday), with shifts between 8:00 AM and 6:00 PM.
Main Duties
The Band 5 Registered Nurse will:
- Act as a link between acute and community services
- Collaborate with the Community Resource Team in Wrexham and Flintshire
- Contribute to the future development of the service
- Facilitate smooth transitions for patients from hospital to community care.

Person Specification
Knowledge & Qualifications
Essential Criteria:
- Registered (NMC) as a nurse.
Desirable Criteria:
- Evidence of post-registration study and training (e.g., POVA)
- Child protection competencies
- Intermediate Life Support certification
Experience
Essential Criteria:
- Pre-registration experience in direct nursing care
- Experience in providing holistic nursing care
- Wild to develop nursing awareness and expertise
Desirable Criteria:
- Clear understanding of clinical governance frameworks
- Ability to implement evidence-based practice
Skills & Competencies
Essential Criteria:
- Strong awareness of contracts of care
- Ability to document accurately and professionally
- Understanding of the consent process
- Basic IT literacy
Desirable Criteria:
- Knowledge of incident reporting policies
Personal/Disposition
Essential Criteria:
- Ability to communicate effectively
- Capacity to work under pressure
Desirable Criteria:
- Fluency in Welsh
