Make a Difference as a Case Manager

Community North is hiring a full-time RN Case Manager.

The Registered Nurse Case Manager plays a pivotal role in patient care coordination. Key responsibilities include:

  • Conducting comprehensive patient assessments, planning, and implementing interdisciplinary care.
  • Supporting seamless transitions across care levels to ensure patient/family needs are met.
  • Coordination with healthcare teams to optimize care continuity and langeímco.
  • Overseeing discharge planning, inpatient/observation unit care accountability, and overall quality management.

Exceptional Skills & Qualifications

Education & Licensure

  • Graduate from a National League for Nursing, Accreditation Commission for Education in Nursing (ACEN), or Commission on Collegiate Nursing Education (CCNE)-accredited nursing school.
  • Valid RN license from the Indiana Professional Licensing Agency (required).

Experience & Certifications

  • At least two years of recent clinical experience or three years of recent hospital case management experience (required).
  • Certification in Case Management (CCM or ACMA) (preferred).
