We are currently seeking a Registered Nurse Case Manager for our Community North region.


Responsibilities

  • Support the healthcare team to facilitate smooth transitions between care levels, benefiting patients and families alike.
  • Provide ongoing clinical support through:
    • Comprehensive patient assessments
    • Careful planning tailored to individual needs
    • Implementation of coordinated care plans
    • Evaluation of ongoing patient management
  • Enhance overall quality of care, promote continuity, and ensure cost-effectiveness by integrating case management, utilisation review, management, and discharge planning.
  • Hold accountability for:
    • Care coordination
    • Discharge planning for inpatient and observation services

Experience in Qualifications

Education & Licensure

  • Must be a graduate of a nursing program accredited by:
    • National League for Nursing
    • Accreditation Commission for Education in Nursing
    • Commission on Collegiate Nursing Education
  • Licensed as a Registered Nurse by the Indiana Professional Licensing Agency (required).

Clinical Experience

  • minimum 2 years of recent clinical experience, or
  • 3 years of recent hospital case management experience (required).

Certification

  • Certification in Case Management (preferred— CCM or ACMA).

Skills & Competencies

  • Strong team-building ability with proven independence and interdependence.
  • Demonstrated flexibility in:
    • Work schedules
    • Site coverage needs
  • Organisational and problem-solving expertise.
  • Open-mindedness to change and readiness to adopt innovative problem-solving approaches.
  • Captain to build rapport with diverse individuals.
  • Advanced computer skills for seamless workflow management.
