The Role
As a member of the BHR Falls Service, you will play a key role in supporting adults with a wide range of needs through tailored therapy programmes. Working under the direct supervision of qualified therapists, you will help maximise patients' independence, improve their functional ability, and support their safety at home or in the community. This is a dynamic and rewarding role that requires a commitment to promoting dignity, safeguarding, and person-centred care.
- Deliver patient-centred falls management programmes in group settings and one-to-one sessions under the guidance of a qualified therapist.
- Monitor patient progress, provide feedback to therapists, and identify when reassessment or additional support is required.
- Manage a delegated caseload, implementing, reviewing, and adapting treatment plans as needed.
- Undertake home visits to review exercise programmes, provide falls prevention education, and support safe, independent living.
- Collaborate with patients, families, carers, and the multidisciplinary team to contribute to care planning and decision-making.
- Maintain accurate clinical records and adhere to legal, professional, and safeguarding frameworks, including the Mental Capacity Act and infection control policies.
- Support service improvements, maintain equipment inventory, and ensure the effective use of resources.
- Promote high standards of patient care in line with local and national policies and Trust procedures.

Essential Skills And Qualifications
- Strong clinical reasoning abilities and the capacity to thrive in a fast-paced, demanding environment.
- Excellent communication skills to ensure timely and holistic patient care.
- Experience in delivering therapy interventions, including group and one-to-one falls management programmes, under supervision.
- Ability to monitor patient progress, provide detailed feedback, and identify when additional support or reassessment is required.
- Competence in managing a delegated caseload and adapting treatment plans to meet individual needs.
- Capability to carry out home visits, review exercise programmes, and provide education to promote safe, independent living.
- Proficiency in maintaining accurate clinical records and working within legal, professional, and safeguarding frameworks.
- Ability to work collaboratively within a multidisciplinary team, contributing to meetings and care planning.
- A valid driver’s licence and the ability to travel within the BHR Borough are essential for this role.

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Goodmayes, Essex.
Probationary Period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
High Cost Area Supplement Outer London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,714 to a maximum of £5,941 p.a. pro rata for part time).
