Vacancy Introduction: Band 3 Rehabilitation Assistant
We have an exciting opportunity for a dynamic and passionate Rehabilitation Assistant to specialise in stroke rehabilitation, and join our supportive, caring multidisciplinary team.
About the Role
The Community Stroke Rehabilitation Team delivers specialist, multi-disciplinary rehabilitation to adults who have had a stroke within a community setting. You will collaborate closely with occupational therapists, physiotherapists, speech and language therapists, psychologists, dietitians, nurses, and therapy assistant practitioners.
The service focuses on goal-driven, time-limited interventions in patients’ home environments or other appropriate settings. The team works alongside Leeds Teaching Hospital Trust and third-sector organisations such as the Stroke Association, providing support across the stroke pathway—from hospital discharge to long-term recovery.
This role is particularly timely as we are implementing a new intensive rehabilitation pathway to enable earlier hospital discharge, complementing our existing community services.
The ideal applicant will have experience working with stroke or neurological patients, familiarity with multi-disciplinary team (MDT) settings, and a keen understanding of community-based health care.

Key Responsibilities
- Implement care plans designed by occupational therapists, physiotherapists, speech and language therapists, and dietitians.
- Participate in joint MDT sessions, especially where two team members’ input is essential.
- Ensure patient-centred, holistic interventions that address range of stroke-related deficits.
- Collaborate closely with partners including Leeds Teaching Hospital Trust and Stroke Association.
