Role Overview
This role of Rehabilitation Specialist will be to deliver Nuffield Health Social Impact Programmes utilising the unique Nuffield Health pathway and championing the Charitable Purpose to ‘Build a healthier nation’. This role will play a crucial part in Nuffield Health’s long-term vision for supporting people with long term conditions with tailored, structured exercise programmes.
Key Responsibilities
- As part of a multi-disciplinary team, the role will require the Rehabilitation Specialist to undertake virtual and in person facilitation of appropriate exercise programming for people with varying abilities & physical function, offering emotional wellbeing support, facilitating individual and group learning around various topics and to promote positive behaviour change, all aiding in an individual’s recovery.
- Identify suitable community locations (such as community centre, sport centre or local authority gym) to deliver the programme from.
- Complete appropriate risk assessments & site evaluation to ensure location safe and appropriate.
- Ensure site is set up & ready to deliver the programme for each session. This includes ensuring equipment stored safely after use.
- Ensure any site specific rules and procedures are followed.
- Recruit participants into the programme, working with site and GPLO’s.
- Monitoring Cohort registration/referrals.
- Manage the transition of phases and overlapped phases.
- Deliver a rehabilitation programme of varying lengths, models (virtual and in person) and specialist areas.
- Facilitate Cohorts of 8 – 13 participants.
- Engage in weekly 1to1 telephone conversations to help set goals, understand restrictions, progress, and offer emotional wellbeing support.
- Deliver live stream classes for the cohorts using MS teams, with people of varying abilities and physical function (where applicable).
- Deliver in person exercise classes, with people of varying abilities and physical function.
- Facilitate individual & groups learning with the aid of the journal and Hub.
- Manage a discharge process when participants have completed their rehabilitation programme.
- Identification of deterioration and further referral, including internal clinical escalations where appropriate.
- Ensure that the participants are working through and completing their support journal, highlighting sections of particular help.
- Ensure that all sessions are scheduled ahead of time and participants receive their sessions plans in advance.
- Deliver programme assessments where necessary.
- Use of the correct entry point scale for all participants, offering progressions and regressions for all abilities.
