Registered Nurse (RN) Case Manager – Transitions Hospice

Key Responsibilities

As an RN Case Manager, you’ll be the clinical foundation for your patients and their families, coordinating care and providing guidance when it matters most. Your work will include:

  • Conducting comprehensive, dynamic assessments to capture evolving patient needs
  • Collaborating with physicians and care teams to create and refine individualised care plans
  • Administering medications, treatments, and interventions as ordered by physicians
  • Ensuring timely and meaningful clinical documentation
  • Serving as the bridge between families, physicians, and community health partners, facilitating seamless communication
  • Educating patients and caregivers to build confidence and empowerment
  • Participating in on-call rotations as per team guidelines

Requirements

What We Look For

  • Active RN license in the state where employed
  • Graduation from an accredited Nursing program
  • Preferred: One (1+) year of experience in hospice, home health, or related clinical specialities
  • Deep-seated empathy for patients and families navigating end-of-life journeys
  • Strong clinical judgment and autonomy for safe, independent fieldwork
  • Exceptional communication skills, adapting to diverse audiences (families, providers, multidisciplinary partners)
