Roald Dahl Paediatric Clinical Nurse Specialist (CNS) for Complex Medical Conditions Transition – Dorset County Hospital
About the Role
The Paediatric Team at Dorset County Hospital is seeking an experienced, motivated, and compassionate nurse to join as a Roald Dahl Paediatric Clinical Nurse Specialist (CNS) for Complex Medical Conditions Transition. This is a Band 7 role, created in partnership with Roald Dahl’s Marvellous Children’s Charity, focusing on supporting children and young people with complex medical needs as they transition from paediatric to adult services.
As a Roald Dahl CNS, you will contribute to a national network of specialist nurses, known for delivering holistic, child-centred care. Based in the Paediatric Department at Dorset County Hospital, you will collaborate with multidisciplinary teams, community services, and regional partners to ensure smooth, safe, supported, and empowering transitions for young people and their families.

Requirements: The Ideal Candidate
- A Registered Children’s Nurse (RSCN/RN Child) with substantial post-registration experience
- Expertise in managing complex medical conditions and confidence in autonomous clinical practice
- Passion for improving transition experiences for young people
- Strong communication, advocacy, and teamwork skills
- Motivation to innovate transition services and incorporate evidence-based practice

Key Responsibilities
- Provide specialist clinical leadership in managing children and young people with complex medical conditions
- Coordinate transition pathways between paediatric and adult services, ensuring seamless continuity of care
- Deliver holistic support to families—addressing emotional, social, and practical needs
- Conduct advanced clinical assessments and tailor personalised care plans
- Educate and train families, carers, and healthcare professionals
- Develop and audit transition services, driving quality improvements across the Trust
- Act as a liaison between hospital providers, community services, and voluntary sector partners
