RRIC Community Practitioner - Occupational Therapist


Join Our Team – Rapid Response & Intermediate Care (RRIC)

Your mission is passionate: Our innovative Rapid Response & Intermediate Care (RRIC) service in Buckinghamshire operates across the county – and now we’re building our Aylesbury Team. Whether you’re a therapist, paramedic, pharmacist, or healthcare practitioner eager to make a difference, this role lets you channel your expertise within a fast-moving, supportive environment.

About the Role

Join a diverse, multi-disciplinary community team where collaboration and clinical leadership mean preventing hospitalisations, empowering recovery, and enabling patients to stay independent in their homes. Your contributions will range from:

  • Early discharge interventions
  • End-of-life support
  • Coordinating interprofessional care pathways

Key Responsibilities

  • Triage, assessment, evaluation: Apply your Occupational Therapy (OT) expertise to patients and carers, ensuring their needs are understood and addressed holistically.
  • Clinical leadership: Plan tailored care, case manage patients through recovery stages, and delegate workloads to the RRIC Team effectively.
  • Leadership across sectors: Work with Physiotherapists, Nurses (all levels), Specialist Paramedics, Pharmacists, and Community Healthcare Assistants to deliver timely, expert-approved care.
  • Pathway development: Research, design, and refine clinical pathways to optimise patient outcomes.
