About the Role

Your Mission

Provide expert interventions and rehabilitation as an integral part of a fast-response team—supporting patients in moments of crisis, enabling independence, and preventing unnecessary hospitalisation.

Your Impact

Help patients stay in their own homes, aid early hospital discharge, and provide end-of-life care. Drive empowerment and recovery as part of a multi-disciplinary community team.

Who You’ll Work With

Collaborate with Physiotherapists, Occupational Therapists, Nurses, Specialist Paramedics, Paramedics, Pharmacists, Healthcare Assistant Practitioners, and Healthcare Assistants.

Key Responsibilities

Core Duties

  • Provide triage, specialist assessment, and care planning for patients and carers.
  • Develop and coordinate care programmes to improve health outcomes and maximize independence.
  • Case-manage patients from initial assessment through to recovery.
  • Assign workloads within the RRIC team to ensure efficient service delivery.
  • Collaborate with multi-professional teams to ensure timely, effective care.
  • Act as a clinical leader, helping refine pathways and workflows.
  • Foster clinical excellence and continuous professional development.
