SEND Transition Coordinator
Location: Northamptonshire – Kettering, Corby and Wellingborough Campuses
Salary/Benefits:
- Annual salary: £30,915–£41,085 (depending on experience and qualifications)
- 27 days annual leave plus bank holidays

About the Role
We are seeking an enthusiastic and experienced SEND Transition Coordinator to join our Foundation Learning and Additional Learning Support department at The Bedford College Group.
As part of a multi-site college network, your role will involve coordinating seamless transitions and providing tailored support for learners with Special Educational Needs and Disabilities (SEND) and Education, Health and Care Plans (EHCPs).
Key Responsibilities:
SEND Assessment & Support Planning
- Conduct comprehensive assessments for learners with SEND and EHCPs.
- Identify appropriate support levels, ensuring provision aligns with individual needs and college policies.
- Oversee the development of personalised support plans.
Transition Coordination & Stakeholder Liaison
- Lead transition planning into, within, and out of the college.
- Liaise with schools, Local Authorities, and curriculum teams to ensure smooth learner progression and holistic support strategies.
High Needs Assessment & Funding Reports
- Perform detailed high-needs assessments for learners.
- Determine appropriate provision and produce professional reports to support Local Authority funding applications.

Collaboration, CPD & Inclusive Practice
- Work closely with teaching staff, parents/carers, and external professionals.
- Provide guidance and continuous professional development (CPD) to foster inclusive teaching practices.
- Promote effective learner support strategies.
Professional Practice, Safeguarding & Compliance
- Maintain ACCURATE AND COMPLIANT documentation.
- Participate in quality assurance and marketing activities.
- Uphold safeguarding, equality and diversity (EDI), health & safety, and CPD responsibilities.
About You
Essential Experience & Qualifications:
- Education/Health/Social Sciences degree (or equivalent) or vocational training in a related field.
- A-Levels (or equivalent), with GCSE English and Maths.
- Strong knowledge of the SEND Code of Practice, local policies in Further Education (FE), and EHCP frameworks.
- Minimum 2 years of experience supporting young people with SEND/EHCPs, including:
- Leading multi-agency meetings (students, parents, professionals).
- Delivering individualised 1:1 and group support.
- Excellent report writing skills and ability to create Detailed financing submissions.

- Communication – Professional, adaptive, and stakeholders-focused (parents, teaching teams, Local Authority).
- Organisational Skills – Ability to prioritise workload, handle confidential documentation, and work efficiently within a busy team.
- Safeguarding, EDI & Professional Values – Committed to safeguarding and codes of conduct.
- Collaboration & Adaptability – Hands-on approach across multiple sites with minimal supervision.
